* Change `block' fields to `context' where appropriate.

* Add two new items to combinations: `frame-adjustment' and
`reuse-existing-frame?'.

* Add `context' field to `true-test' node.

* Add new node type: `stack-overwrite'.

* Delete `node/offset'.  Offsets are now stored in reference contexts.
