First batch of changes to run scheme:

Add ASM_REGISTER_BLOCK definition and ASM_RESET_HOOK.

A6_OFFSET was incorrect.  It corresponded to scheme_to_interface, not
trampoline_to_interface.

Change the definitions of FORMAT_BYTE_MUMBLE and FORMAT_WORD_MUMBLE to
not depend on signed numbers being correctly cast.
