Generate most of the Makefile.in files from fragments and boilerplate.

Generate most of the Makefile.in files from fragments and boilerplate.
Replace "top_builddir = ." with "top_builddir = @top_builddir@" in all
configured makefiles, since autoconf now provides a reasonable default
for these.
